Dear Garden Pediatrics Families,

First, I want to sincerely thank each of you who took the time to complete our recent survey. Your feedback has been invaluable as we work to create a care model that continues to meet the needs of our families while sustaining the high level of care you deserve and expect. Additionally, thank you to all the families who reached out individually voicing their encouragement and support for the work that we do. It really has meant so much during this time and while it has not been possible to respond to all your messages yet, I so hope to reach out to each of you.

Based on your responses - and with our goal of including as many of our current families as possible - we are excited to announce that Garden Pediatrics will now offer three care tiers beginning in May, with new enhancements to better support your child’s health.

UPDATED MEMBERSHIP TIERS

Tier 1:

Complete Care

$2,300* /yr or $200 /mo

Siblings - $2000* /yr or $175 /mo

*discount if paid in full

Ideal for families with HMO plans (such as Kaiser, HMOs, IEHP, high deductible plans or RiskManagement), those without insurance, or anyone seeking the highest level of access and simplicity.

No insurance billing. Everything is included for in-house services (except cost-based specialized PCR testing and vaccines). Vaccines would be either billed by Vaxcare if you have a PPO and are going this route or could be received through county clinics if no insurance or received from your HMO.

• Copays paid by Garden Pediatrics

Direct communication with your child’s primary provider (Dr. Carter or Erin Nolan, PNP) via text or phone

After-hours messaging available with $50 surcharge per message between 9pm–7am

No additional fees for new patient registration, paperwork, referrals, or prior authorizations

• Priority same-day or next-day appointments

This tier offers the most comprehensive and seamless care experience, eliminating surprises and prioritizing your time and peace of mind.

Tier 2:

Hybrid Concierge

$1,700* /yr or $150 /mo

Siblings - $1,400* /yr or $125 /mo

*discount if paid in full

Perfect for families who want to keep using insurance but also value enhanced access and reduced administrative burden.

• Insurance will still be billed

• Copays paid by Garden Pediatrics

Direct communication with your child’s primary provider (Dr. Carter or Erin Nolan, PNP) via text or phone

After-hours messaging available with $50 surcharge per message between 9pm–7am

• No fees for form completions, referrals, or prior authorizations

• Priority same-day or next-day appointments

Even if your family only comes in a few times a year, this tier supports all the between-visit care we provide - nurse calls, email advice, medication requests, and referral help - that insurance doesn’t cover.

Tier 3:

Traditional Insurance with Service Fees

No monthly fee

Families may continue using insurance, but additional service charges will apply to help cover costs not reimbursed by insurers. If you don’t sign up for a concierge tier, these charges will now apply:

• $50 per referral requiring extra administrative work*

• $25 per prior authorization (insurance or pharmacy)**

• $25 per form (school, camp, sports) not completed during a visit.***

• $50 rush fee for forms needing 1-day turnaround

*Everything needed for referrals will be given at time of visit including recommended options for places to go and contact information for parent to make the referral. If more is required from the referral center then the patient family will reach out to us and charge assessed.

**if requested by insurance company and parent wants us to do

***FMLA and like forms will require additional visit

Why These Changes?

Over the past several years, insurance companies have pushed more administrative burden

onto practices without paying for the time, staff, or systems required. Services like referrals, forms, and authorizations - though essential to your child’s care - are not reimbursed. This model is no longer sustainable.

By enrolling in one of our concierge tiers, you help ensure we can continue offering the time, access, and personalized support that makes Garden Pediatrics special. We hope you’ll consider joining, especially the Hybrid Concierge tier, which allows us to serve more families while improving financial stability for the practice.

We Remain Committed to You

Our mission has always been to provide a warm, personalized medical home where families feel seen and supported. These updates ensure that mission continues, while keeping our care model fair, transparent, and sustainable. We are honored to be part of your family’s journey and look forward to growing with you.

Warmly,
